Use these general checklists to prepare for enrollment. The final requirements may vary by program, campus, and school year, so confirm the official list with the CELTECH College Admissions Department.

Senior High School

For incoming Grade 11 students.

  • Report card / latest school record
  • PSA birth certificate photocopy
  • Certificate of Good Moral Character
  • Recent ID photos, if requested
  • Additional school forms, if required

College Freshmen

For students entering college for the first time.

  • Senior High School report card / Form 138
  • Certificate of Good Moral Character
  • PSA birth certificate photocopy
  • Graduation certificate or school records, if requested
  • Recent ID photos and application forms, if required

Transferees

For students transferring from another college or university.

  • Transcript of Records or available academic records
  • Honorable Dismissal / Transfer Credentials, if required
  • Certificate of Good Moral Character

  • Subject descriptions for credit evaluation, if requested
